Transnasal Transphenoidal Skull Base Surgery – Extended Endoscopic Approaches
The endoscopic endonasal approach to the sellar region is a recent evolution of the conventional transsphenoidal technique performed with the operating microscope. But no longer is it considered a complement to microscopic surgery; rather it has now become “pure” pituitary and skull base endoscopy – the term “pure” being applied to a surgical procedure in which the endoscope is the only optical device used.
The endoscopic/endonasal approach has several advantages:
- It makes the nasal phase of the transsphenoidal microsurgical procedure – involving the creation of a submucosal tunnel – unnecessary.
- It affords a wider exposure of the operating field and offers the option of advancing the scope toward the anatomic target area, which no longer has to be viewed from the distance.
- It allows the sellar lesion to be removed more safely, thanks to a close-up view of the anatomic structures. This added safety, in turn, reduces overall complications.
- It provides a better aesthetic result since the procedure is less traumatic.
- The procedure is less time consuming and the hospital stay is shortened.
