Microneurosurgery and Endoneurosurgery
Complementary techniques for minimally invasive transcranial operations with a special instrument set according to Umberto Godano, MD, Azienda Ospedaliera G. Brotzu, Cagliari, Italy
Endoscopic visualization complements microsurgery and can be employed at various stages of an operation.
The main advantages of endoscopy are:
- A better direct exposure due to a more effective illumination and the enlargement of the endoscopic picture at a high resolution and with a wide field of view.
- The ability to use the endoscope to investigate regions that cannot be viewed directly with a microscope.
KARL STORZ created a special instrument set for transcranial operations. This set enables the surgeon to work with both hands because the endoscope is fixed to a holding system.
The main features of the transcranial operation set are:
- Fine design for delicate dissection
- The instruments are “stick-shaped” with a low profile to accommodate the narrow access channel.
- The ergonomic distal handle for a controlled motion of the device.
- A diameter of 1 mm for handling very delicate structures (slightly flexible)
- A diameter of 2.2 mm for solid lesions, these instruments can also be used for routine microsurgical activities
- The use of rigid endoscopes with a diameter of 2.7 mm, with various lengths (11 or 15 cm). There are two different angles of view 0° and 30°
- The eyepiece of the endoscope may be straight or angled. Endoscopes with angled eyepieces are usually recommended because the instruments are easier to manage.
- Easy to handle and learn
It is recommended to use a high resolution camera (KARL STORZ IMAGE 1 HD).
